AN OVERVIEW OF OUR APPROACH

Initial Consult – You can fill out out our referral form to help us. We will then talk with you or the person you support. Sometimes, you might be seeing who is the “best fit” for you. All referrals will be discarded should you choose another provider.
Meet with you / Collaborate – We will meet with you, service provider/s (if relevant)  and the person you support. We will listen and collaborate with you to gain insights about your needs.

Information Sharing – We will exchange information with you in alternative formats if needed. We will provide information about PBS, other relevant information (e.g. specific conditions) and our methods of work. With your consent, we will exchange information with services and people such as service providers, schools, relevant health professionals and allied health professionals to help support our work.

Multi-Disciplinary approach – We highly value the input and specialist skills of other professionals that may be involved in supporting the person. This means we will collaborate to the greatest extent possible with other professionals as listed above in collaboration with you.

Observation and Data Collection – We will support you and others you nominate to understand data and it’s use in collecting evidence. We will discuss the need and purpose of observations and interactions with person needing support. We will conduct a Functional Behaviour Assesment (FBA). This helps us to determine what the function of a behaviour might be to create individualised, proactive strategies. We will explain this process to you.
Plans and Strategies – We develop a Person-Centred Positive Behaviour Support Plan based on the evidence collected. This may be an Interim Plan to start with. We may recommend strategies to assist in managing risks of behaviour that has potential to cause harm to the person or others in different formats. We may assist in developing a Person-centred plan or Support Plan that  has a “whole of life” perspective.Education is part of every step of our approach throughout the process. We may provide Skill Development materials to support the learning of new skills and assist in implementing these. We will provide materials, tailored to your individual needs to support you, individuals you nominate and/or teams. This may be resources, tools or face to face training for people who support the person.

On-going Support – We are available to support you should you have any queries.
